Oxytocin is a nine amino acid hormone made in the hypothalamus and released by the pituitary. It has been an approved medicine for decades in a specific form, given intravenously in hospitals to start or strengthen labor and to control bleeding after birth. That use is well established and uncontroversial.
Everything else about oxytocin is far less settled.
It picked up the nickname the love hormone from research suggesting it shapes bonding, trust, and social behavior. That framing spread widely, and it is now sold as a nasal spray for social anxiety, connection, and mood.
The problem is a practical one that is easy to overlook. Oxytocin does not cross from the bloodstream into the brain well, which is why the nasal route is used at all, on the theory that some reaches the brain directly through the nose. How much actually gets there is still debated, and it is one reason results have been so inconsistent.

Oxytocin is a case where an appealing story ran well ahead of the evidence, and the evidence has been catching up in a disappointing direction.
Early studies suggesting effects on trust and social behavior drew enormous attention. Larger and better controlled trials have been much less encouraging. A meta-analysis of 12 randomized controlled trials covering 498 people with autism found no significant effect on social impairments overall, although a benefit did appear at the higher dose of 48 IU daily. A large trial in adults could not recommend continuous intranasal oxytocin at the doses and duration tested for core social symptoms, while suggesting possible benefit for repetitive behaviors.
The pattern is small effects, inconsistent between studies, and poor replication. That is the signature of a real but weak effect, or of no effect at all being detected unreliably.
There is also a mechanistic problem worth knowing. Laboratory and animal work suggests that repeated dosing causes oxytocin receptors to internalize and desensitize, with receptors in the amygdala particularly affected. If that holds in humans, daily use may actively work against itself, which would be the opposite of how most people assume it should be taken.
